Job Description Company - MyCHN Position - Women's Health Provider Location - Scarsdale Family Health Center; 10851 Scarsdale Blvd Suite 160, Houston, TX 77089 MyCHN is a nonprofit organization providing Medical, Dental, Behavioral Health, and Pharmacy services to communities in the Greater Houston and Gulf Coast area. We were founded in 2008 with one provider, three exam rooms and a big dream. Today, we have over 100 providers and more than 15 locations. Job Summary: The Women’s Health Provider is a nurse practitioner or physician assistant that provides specializes comprehensive healthcare for women and children across the lifespan. This provider can apply advanced knowledge, skills, and expertise to the planning and implementation of safe and quality patient care with an emphasis on conditions unique to women and children. They are to collaborate with physicians and be committed to ensuring a positive patient experience while following patient-centered medical home (PCMH) guidelines and principles. Duties and Responsibilities: - Applies advanced specialty knowledge of Women’s Health, clinical skill, and expertise to assess patients and develop a list of actual and potential needs/diagnoses. - Assesses the physical and mental condition of women and children by performing and obtaining comprehensive, problem-focused physical examinations, and medical histories. - Identifies risk factors in Women’s Health and Pediatrics through discrimination of normal and abnormal findings obtained through health history, physical examination, laboratory and/or radiological test results, and behavioral assessment. - Perform obstetrical, gynecological, and pediatric procedures while incorporating health education and prevention. - Focuses on women’s prevention care, primary care, acute illnesses, and chronic conditions. - Able to prescribe and/or recommend appropriate forms of related therapeutic treatments. - Must maintain patient documentation in a prompt and comprehensive manner (such as patient visits, laboratory/radiology results, referrals, pharmacy requests). - Performs other related duties as assigned by management.
