disabled patients within their homes Monitor and report on patient
condition and document patient care services Perform diagnostic
tests and collaborate with fellow members of the healthcare team
Follow care plans and ensure consistency of care Educate patients
and family members on proper home health care strategies and
procedures Strong clinical skills in a home health setting
Proficiency in administering medication and providing wound care
Excellent communication and interpersonal skills Ability to work
independently with minimal supervision Medical Insurance Dental
Insurance Vision Insurance Life Insurance Current LVN license in
the state of practice CPR certification
