Job Description Job Description POSITION SUMMARY Plans,
coordinates, and provides hospice nursing care in accordance with
the patient’s individualized plan of care. POSITION
RESPONSIBILITIES Coordinate hospice care in collaboration with the
interdisciplinary group (IDG), patients, and family members to
ensure patient and family needs are met. Evaluate the effectiveness
of hospice services to the patient and family on an ongoing basis.
Perform initial, ongoing, and informational visits as indicated.
Utilize the nursing process to provide safe, professional nursing
care. Communicate with the attending physician and Hospice Medical
Director regarding changes in patient condition requiring new or
modified orders and updates to the plan of care. Coordinate patient
services with IDG members and contracted providers as needed.
Complete accurate, timely, and relevant clinical documentation in
accordance with agency policy. Participate in interdisciplinary
group (IDG) meetings. Ensure patient plans of care remain current
and reflective of patient needs. Perform on-call duties, including
holidays, per agency policy and operational needs. Maintain and
enhance clinical competence through in-service education,
continuing education, workshops, conferences, professional
involvement, and self-directed learning. Educate patients and
family members regarding the dying process, plan of care, and
healthcare decision-making. Provide dietary counseling appropriate
to the patient’s condition and end-of-life needs. Perform death
visits in accordance with agency policy. Participate in discharge
planning as appropriate. Maintain current knowledge of
pharmacologic therapies relevant to hospice care. Comply with all
federal, state, and accreditation requirements, including Medicare
and Medicaid regulations. Assist with coverage for other nursing
staff as requested by a supervisor. Conduct comprehensive initial
and ongoing patient assessments. Obtain and document medical
history from the patient and/or family, focusing on the current
condition. Perform physical assessments, including vital signs,
mental status, appetite, and dietary intake. Assess patient,
family, and home environment to identify education and support
needs. Evaluate available caregiver support and determine required
services. Demonstrate knowledge of hospice levels of care and
appropriately recommend transitions when indicated. Explain nursing
and agency services to patients and families as part of care
planning. Develop, implement, and maintain the comprehensive plan
of care in collaboration with the IDG. Provide skilled nursing
services, treatments, and preventive procedures as ordered by the
physician. Observe, assess, and report patient responses to
treatments and medications, including physical and emotional
changes. Supervise and evaluate Hospice Aide services at least
every 14 days and as needed. Document nursing services provided,
changes in patient condition or environment, supervisory visits,
care plan revisions, and discharge summaries. Evaluate the
effectiveness of nursing care provided to patients and families.
Communicate patient care concerns and proposed solutions to the
supervisor. Provide guidance and supervision to LVNs at least every
30 days. Collaborate with other healthcare agencies to ensure
continuity and comprehensive care. Participate in staff development
and team meetings. Prepare and maintain the Hospice Aide plan of
care. JOB CONDITIONS Work environment may be stressful. Must be
able to access patient homes, which may not be wheelchair
accessible. Must possess adequate vision, hearing, and physical
dexterity to perform patient assessments and care. Physical
requirements include walking, sitting, standing, stooping, and
lifting or repositioning patients as needed. EQUIPMENT OPERATION
Use of computer systems, telephone, thermometer, blood pressure
cuff, glucometer, penlight, hand hygiene supplies, and standard
medical and office equipment. CONFIDENTIALITY Has access to patient
medical records and is required to maintain confidentiality at all
times. QUALIFICATIONS Graduate of an accredited school of nursing.
Current Registered Nurse licensure in the state of practice.
Minimum of one (1) year RN experience; hospice experience
preferred. Strong assessment, critical thinking, and
problem-solving skills. Effective organizational and
time-management abilities; knowledge of case management principles.
Valid driver’s license with reliable, insured transportation.
Excellent verbal and written communication skills. Unrestricted RN
license and demonstrated competency in medication administration,
medication reconciliation, and nursing procedures within state
scope of practice. All required competencies must be validated
